36 years old female presented with severe abdominal pain with jaundice with weakness being wheelchair bound.Her PETCT showed multiple liver metastasis with liver full of disease.However biopsy and immunohistochemistry confirmed high grade Burkitts lymphoma , her MIBI index was 98 % ,which is very aggresive lymphoma which is rapidly fatal if not treated early.She was urgently started on cytoreductive chemotherapy along with TLS prophylaxis of inj rasburicase ,and later intensive chemotherapy along with targeted therapy ( R- Dose adjusted EPOCH).She had rapid symptomatic relief and all liver lesions resolved and patient had complete resolution after 6 cycles of Chemotherapy.High grade lymphomas like Burkitts lymphoma are very aggressive but at the same time can be extremely responsive to high intensity treatment.

How is Immunotherapy given?

Immunotherapy is generally given as infusions in the vein. They enter the blood circulation and influence the immune cells to attack the cancer cells wherever they are in the body. Immunotherapy treatment requires a daycare admission in an Oncology day care unit, and patients are discharged the same day. Immunotherapy is generally given at regular intervals of 2 to 3 weekly for prolonged duration often for many months until the time it is no longer working. Immunotherapy drug doses are generally calculated based on the patient’s weight or come as standard recommended dosing depending upon the drug.

What are the side-effects of immunotherapy?

Since immunotherapy can also affect some healthy cells, it can lead to side effects and hence should be taken under the care of doctors who are trained in delivering the treatment safely and do it regularly. Different Immunotherapy drugs have other side-effect profiles, and the patient should discuss it with their cancer specialist in detail before starting the treatment. Since the healthy cells can repair themselves, most of the side-effects will improve or go away with time or by giving medicines that suppress the immune cells.
